We love to receive positive feedback from our clients, their families or any related stakeholders. This lets us know what we are doing right and ensures we continue to provide supports and services in a way that meets your individual needs.

We encourage you to raise concerns with your therapist/s in the first instance where possible so they can be addressed directly. This form is available for use if you don’t feel comfortable to do this, or where your concerns have not been addressed via this communication. The information you provide will be used to resolve the issue in the first instance then also to direct service improvements if required. Information will also be recorded on WizeTherapy’s complaints register. We advocate for open and respectful communication between all parties throughout the complaints process as we work together to achieve a satisfactory outcome for you.

If you have a complaint specifically about the way that we provide services under the NDIS, you may wish to lodge a complaint directly with the NDIS Quality and Safeguards Commission. In particular, the Commission wants to hear about;

• NDIS services or supports that were not provided in a safe and respectful way.
• NDIS services and supports that were not delivered to an appropriate standard.
• How a NDIS provider has managed a complaint about services or supports provided to a NDIS participant.
